Inserting New Citations Always Number 1

I am using Vancouver Style citations - whenever I add a new citation it always adds it as "1", even though I already have over 40 citations in my document. Has anyone else come across this issue and have a solution? I tried changing citation style to a different numbered style (Nature), and it did the same thing. Thanks.
