When/what installs zotero.dotm in the Word startup folder?
I've had a user on a new laptop build report the Zotero ribbon wasn't visible. Turned out the zotero.dotm file was missing from their Word startup folder. I got a copy from a working laptop and saved it to the startup folder - that fixed it.
However, I'm interested in why it wasn't there. When/what puts a copy into this folder? I don't think it's done at installation as I tend to install apps using a different account during set-up.
I first tried disabling and re-enabling the add-in through the app but that didn't help so it's some other step that adds it.

Slight bug here. On my laptop, Zotero ribbon is working fine: https://i.imgur.com/1fB4mMu.png
However, when I open that tab on preferences, it says it's not installed: https://i.imgur.com/peErGr9.png
The add-in on this system will have been created when Zotero was first launched. If you re-install it, close and re-open Zotero, it then says it's installed: https://i.imgur.com/M2MEqkx.png
