Item still associated with tag after tag removed

I have a couple of items in my library that used to have a tag associated with them. I removed the tag from these items, but the tag is still associated with other items in my library. When I click on the tag, though, it still retrieves these old items. They're kind of greyed out, but they're still there.

Does anyone know a) what's going on, and b) how to fully dissociate this tag from these items?

    Sounds like the tag is _also_ associated with an attachment or note of that item (they can be tagged separately from top-level items)

    Oh, you're totally right. No idea how that happened as I haven't deliberately added tags to any notes, but that's a mystery for another day. Thanks!
