Won't include one book in Bibliography?

I can't seem to get Z. 5.0.54 to include one book in the bibliography. This work is cited in a note and in the text but it doesn't show up in the bibliography (Chicago author-date); I've reentered the item, re-entered the citation, and changed its position. Mac OSX 10.10.5; 2011 Macbook pro; MSWord for Mac 2011 14.7.2.

Weird. It's a fine book!

    Does that book show up if you cite it in the main body of the text rather than a footnote?

    No it didn't. But -- UPDATE -- I completely deleted the item (the missing book) and emptied the trash as well as completely eliminating it from the text and notes of my manuscript. Then I re-entered the item and the note/citation and now it looks fine.
