Preview is rather strange with style with no citation numbers

Comment line 195 back in and it previews okay:

But it previews fine in Word with the line comment out so not too worried:

Have find a few instances where the preview window displays different output to when used in Word.

You should remove second-field-align="flush" in the style. That does exactly what you're seeing in the comment, i.e. put the first element (which in your case is the entire citation) into the left margin.

Thanks for this - I assumed it was something to do with indentation/alignment. What my client actually needed was to replace second-field-align="true" with hanging-indent="true". The fact that it was rendered correctly in Word with the former switch looks more like an accident.
