ZotFile and the curious case of extracting any annotations at all

I have Zotero 5.0.27 and ZotFile 5.0.6. The Zotfile is using pdf.js as the libpoppler option is greyed out and can't be used.
On the Zotfile homepage it says it is able to extract annotations and hilights from PDFs.
I cannot figure for the life of me how to extract them! I have tried reindexing all files etc. but never do any notes or anything appear in Zotero. The PDFs do contain hilights and post-it notes when they are opened, but nothing gets transferred to Zotero.
I have made the annotations in Adobe Acrobat Pro DC.
I am quite sure this is just my own stupidity but I have tried going through all the menus and read ZotFile documentation and can't figure out how to do this.

Upgrade Storage
In general, you always want to be working with regular Zotero items, not the bare attachment files. This will store the items' bibliographic metadata and enable most of Zotero's functionality. See here:
