Style Request: American Medical Association, Western Journal of Emergency Medicine

The Western Journal of Emergency Medicine is asking for certain modifications to the AMA citation. Can anyone help me modify the "American Medical Association (no URL)" so that if there are only two authors, replace comma between names with “and”? However, if there are 3 or more authors, the journal does not want the "and" there.

I've figured out how to modify it with the other-things that I need, but I can't figure this out.
