reference format for 'Pediatric Nephrology' - HELP!

Hi everyone,
I need to format my reference list for the journal Pediatric Nephrology as follow

Gamelin FX, Baquet G, Berthoin S, Thevenet D, Nourry C, Nottin S, Bosquet L (2009) Effect of high intensity intermittent training on heart rate variability in prepubescent children. Eur J Appl Physiol 105:731-738

with especially these instructions
a) complete lists of author names, ie DO NOT abbreviate with 'et al'
b) complete page numbers, ie 441-449, NOT 441-9
c) do not include the month
d) ensure all journal titles are correctly abbreviated in line with Index Medicus, as used on PubMed

And I can't find something that fits in the repository
