"EBSCO Record" appearing as separate item in bibliography

I'm trying to generate a bibliography from a collection and the output gives me all the items in the collection but also a string of items beginning EBSCO Record (nd-... [Example output text below]. It looks like there is one for each item in the collection that did not have a pdf attached.

I've tried bibliographies in Chicago and APA and I've tried generating bibliographies from other collections with the same results.

    Do you have items expanded when you do select all? This looks like you're creating a bibliography from an attachment, which could happen that way. (You can collapse all items in the center panel using the - key on your keyboard)

    That certainly wouldn't be what I would expect to happen.
