Note that the et-al-rule from AMA is as follows If you apply this rule than this will still result in "list 3 authors followed by et al". Actually, this sentence is not really saying anything about how to deal with 4 or 5 authors.

zuphilipI can confirm that it doesn't use italics. Thus, it looks that these are the obvious differences maybe even all differences. I can do a PR for this journal. @adamsmith: Okay?
