AMA style - without issue in citation
Hello!
I have to inset for a journal "the latest edition of the American Medical Association (AMA) Manual of Style"...When inserting reference numbers remember that the reference number appears after periods and commas, but before colons and semicolons.
I use AMA style but the numers appear before periods and commas.
How can I find this style?
I must look like:
Campbell SK, Hedeker D. Validity of the Test of Infant Motor Performance for discriminating among infants with varying risk for poor motor outcome. J Pediatr.(italic!) 2001;139:546-551.

Could you please help me with another problem?
For my journal I do not need in my ciatation the issue (10); only 11:1305-1319. I get following citation with my inserted AMA-Stlye:
Cole TJ, Green PJ. Smoothing reference centile curves: the LMS method and penalized likelihood. Stat. Med. 1992;11(10):1305-1319.
Does there exist an extra AMA Style without the issue in the citation?
