Upgrade StorageStandalone create bibliography box is blank

I was using v4.0.17 and one day I could no longer create bibliographies/citations - the window pops up but the citation style dropdown is blank, and there is text missing in the actual window. When I click OK I get the error message [JavaScript Application]: "An error occurred generating your bibliography. Please try again."
So I upgraded to v4.0.28, and when that didn't work I uninstalled the program completely (add-in, extension, etc>) and reinstalled latest version but it's still happening.
I'm using Windows 8 with Chrome and Word.
