Title & short title in one citation simultaneously?

I'm trying to do something like the following for the first instance of a citation:
In re Kuriappan P. Alappat, Edward E. Averill and James G. Larsen (1994) 33 F.3d 1526 (“Alappat”)
That is, the long title and the short title both need to appear in the one citation. But for some reason this returns a blank, even when there is definitely a value in the short title field:
<text variable="title" form="short" prefix=" (" suffix=")" quotes="true"/>
On the other hand, if I just put the same value in the Extra field, as follows:
<text variable="note" prefix=" (" suffix=")" quotes="true"/>
I get ("Alappat") as required. Is this because there is a presumption that each field will appear only once in the output style, or am I just doing something wrong?
