endnotes unlinked randomly

Hi, I have a document that I've been working on and off for the past couple of months. Today I added three endnotes to it without problem. Then when I went to add the next one -- it started over from with numbering from 1 additionally when I click on other endnotes to edit them, the dialogue box is blank -- as if there had been no zotero reference in the citation (even thought it is one). help!
