[MLZ] "near-note-distance" parameter
Does "near-note-distance" count only footnotes/endnotes with unique citations in them? It seems to ignore footnotes/endnotes without citation, as well as citations expressed with "Id".
If the purpose of "near-note-distance" is to prevent a situation where the reader has to flip through too many pages to reach the prior citation, I think all footnotes with/without unique/subsequent citations in them should be counted.
