I get message "Last name 'xxxx' too long" but

(a) 'xxxx' is not in the 'Last name' field - it is in the title field
(b) Even when I delete the item with 'xxxx' in its title field, I still get exactly the same error message citing 'xxxx'

Any ideas or advice please?

    delete the item, empty the Zotero trash (bottom of the left hand panel) and try again.
    After deleting the item, do search for xxxx again to make sure the title didn't get into the last name field of another item. It wouldn't seem likely that Zotero actually gets the field in that message wrong (though I believe there is also a character limit on titles).

    If none of that helps, provide a debug ID for the process of a manually triggered sync:

    Great, Adam - clearing the Trash seems to have made it work. (Why then such a peculiar error-message I wonder?)

    I'd guess that the item in question, with that value in the Last Name field, was already in your trash. I just tested, and I get the correct message if the title field is too long. (The character limit on titles is also much longer than names.)
