[citeproc-bug] Disambiguation "by cite" falsely kicks in for authors in same work

Sorry to bother you Frank - I think this is a bug:
When using givenname-disambiguation-rule="by-cite"
a single work by two authors with the same name (i.e. Collier and Collier 2002) still gets disambiguated (i.e. R. B. Collier and D. Collier 2002)
That clearly shouldn't be the case.

Thanks. Made a test, reproduced it, and have applied a fix. Full test suite is running now, when it's finished I'll check in a new processor version.
