Problem with Harvard Styles (Institute of Physics (Harvard))

In my text I get references like (G. Pfur and Lop 1999) instead of (Pfur and Lop 1999)
Another example: (John Doe, Smith, et al 2009) instead of (Doe et al 2009a).
What is happening here?
-> Yes there are more Entries by John Doe in 2009 -> but they should be referenced to as (Doe et al 2009a).

Okay, maybe the problem is, that the style does not refer like (Doe 1999, 2009a, 2009b) but considers the next authors instead of a letter in order to make a reference unambiguous.
How may I change this behaviour in the CSL?

if you can provide documentation (either instructions from the style guide or an example from a published work) that the current behavior is incorrect for IOP I can change this globally.

From the IOP style guide:
...If you refer to different works by one author or group of authors in the same year they should be differentiated by including a, b, etc after the date (e.g. 2001a)...

the question is if you have

Smith, Meyer, Adams 2009
Smith, Doe, Clinton 2009

should they be (Smith et al. 2009a, b) or (Smith, Meyer et al. 2009; Smith, Doe et al. 2009)

The current style will already give you Smith et al. 2009a,b if you have two citations from an identical group of authors.

No dot after et al and in italics (following the style guide).

(Doe 1992a, 1992b) instead of (Doe et al 1992a, Doe et al 1992b)
(it is not explicitly required by the style guide) -> how may I establish this?

However, the list of references does not include url of web pages and date accessed. could you help sort out this?

seneca - I can't find any reference on IOP on how to cite web pages - do you have a section of a style guide or an example from an IOP journal that we could serve as an example?

Thanks jfr. I fixed it. A line was missing in the citation style code you posted at gist.github.com. Inserting this <text prefix=" " macro="access" suffix="."/> after line 191 solved my problems.

OK, this is now fixed on the repository according to the example provided by jfr above (i.e. no access date and "Online: " before the URL.

Put it in as a book section, with Konotey-Ahulu as the author and the other as editors. If that doesn't help you'll need to be more specific about what the problem is.
