does not import into selected library

When I import a citation in Zotero 2.1.6, the item does not appear in my selected library, but in a location titled "Unfiled Items". I then have to copy it into the desired library. Why is this? This is new for me for version 2.

Is this before or after opening Zotero in a new session? For me, newly downloaded items only land in "Unfiled Items" if Zotero hasn't been opened since a restart (or I have My Library selected in the left panel, of course).

I'm not sure what you mean by "opening Zotero in a new session".
So for instance, I open FF (it happens to be the first time, since I booted up), then open Zotero (by clicking on the Zotero button in the bottom FF status bar). So I then have a active Zotero window (with 3 panels) "on top of" the FF window, about 1/3 the height of the FF window. In FF I go to some resource I for which I'll download the citation info. In the left panel of the Zotero window, I've selected my specific library of choice under "My Library". Next I download (or export) the citation info, and it's placed in the library I expected (!!) -- this seems new.
Okay, so I'll repeat this with another citation; this time it does what it was doing before -- it placed the citation in "Unfiled items" instead of my selected library.
So what exactly are you suggesting I need to do? Close Zotero after each citation download and reopen for the next download? I'll check into this some more.

Provide a URL for an item that doesn't get saved to the selected collection.

I don't know that it's specific to the URL, b/c it works fine with the first import but on subsequent imports, goes to "Unfiled items".

I've run into the same problem twice now (both with my at home use and my use on campus, unsynced, new install).

Is it dependent on citation quantity? I was working on a meta analysis and some time around the 200th reference whenever I added a new citation it placed it into unfiled items by default, even when I had the library I wanted it to end up in selected.
