Both manually creating and importing items creates two entries. One is a ghost entry that disappears after selecting a collection not containing the item and then returning to a collection in which the item exists.

dstillman Zotero TeamRestart in Troubleshooting Mode (Help → "Restart in Troubleshooting Mode…”), which temporarily disables all plugins. If that fixes it, you'll need to identify the plugin that's causing it and then report the problem to the plugin's developer. If not, we'd want to see a Debug ID for reproducing it, as well as a screen recording (which you can link to or send to support@zotero.org with a link to this thread).
