PubMedCentral Access through Library Server

Is there a way to link requests through my libraries server to PCM for a specific journal article based on a RSS link in something like Google Reader? The identifier provided by the RSS is not the PMID# but rather a journal specific such as R99 for the journal Critical Care. I've tried changing the proxy to /journal/etc.... as per URL suggestions found @ pubmed central help but no luck.

Can anyone provide any insight?
