In NeoOffice go to 'Tools' -> 'Extension Manager'. Click on My Extensions and the click the 'Add' button on the right. Use 'Browse' to locate the 'ZoteroNeoOffice.oxt' file and that's about it. Re-start NeoOffice!
So once I've installed the extension, where is it accessible within Neo Office? Is there a toolbar? I looked at the screencast but those icons aren't showing up.

Yes, craniac, you simply point the Extension Manager to the zipped file you downloaded. (Do not unzip it first.) Extension manager does the rest, automatically unzipping the file and installing the plug-in. Once Extension Manager is done, restart NeoOffice.
Go to View > Toolbars and activate the toolbar called "Add-On." You will now have a free-floating toolbar with the Zotero functions.
