I am curious what the thought is behind having two places to enter Tags for an item?
When I select an item, the right pane shows metadata aspects of interest, including Tags. Then, when I click on the pdf file within the item, a separate Tags item appears in the metadata pane to the right.

dstillmanGenerally you want to assign tags to the parent items or to annotations in the reader. There are some more esoteric workflows where assigning tags to attachment items makes sense, but I wouldn't generally recommend it.
