no short titles in footnotes, what am I doing wrong?

I am using Chicago Manual 17th edition full note format, so the footnotes should include the full citation in the first footnote and then the author's last name + short title in all subsequent footnotes. This doesn't appear to be working in my document as you can see in the image:

Footnote 8 is first mention, footnotes 9 and 10 are incorrectly formatted, footnote 12 is what I actually want for the short (subsequent) form.

There are differences in the code displayed when I hover over the different citations but I am not knowledgeable enough to understand how this happened or how to change it. This screenshot shows the incorrectly formatted citation's code (author only):

Does anybody have a clue as to how this could have happened and how I could fix it?
  • That's correct Chicago style for _immediately_ subsequent notes (what used to be ibid). If you always want short titles, there's a Chicago variant with something like "short titles in subsequent" in the name that does that
