Plugin not creating tab and buttons on Word365 ribbon - Solved

I have followed all of the troubleshooting steps listed in you documents including all the manual steps.
I can see in Word that the add-in is loading fine and not being blocked by macros security. To prove this I manually created a ribbon tab and could add the Zotero macros to it. These macros work with the main applications as expected.
It would seem there is something in our environment that maybe stopping the load to the ribbon but I can't figure out what that would be. I am looking for other ideas?

If your Normal.dotm has been customized, Word may not be able to add tabs. You can reset your Normal.dotm template by moving it somewhere else (like Desktop) and restarting Word. This will remove any customizations you've done, but might allow the Zotero tab to appear.

I have actually found the issue and sharing here as it may help others.
Adding this Intune policy stops the add-in from being added to the toolbar (CIS L1 setting)
The policy sets this regkey
HKEY_CURRENT_USER\SOFTWARE\Policies\Microsoft\office\16.0\common\toolbars\word\noextensibilitycustomizationfromdocument = 1
