Inconsistent "et al" behaviour?

I've noticed that the very same reference to a book section (APA-6) is displayed in two different fashions. One produces all three authors followed by a year, the other produces the name of the first author, followed by "et al" then followed by the year.

I'd like all references to be uniform across the paper, of course. How do I achieve that without resorting to manually editing the reference?
  • I don't have the style manual here right now, but as far as I remember, according to APA guidelines, the first time a work is cited up to 5 authors should be listed. On subsequent citations, et al. is used after the first author for all citations with more than 2 authors. That's how it's coded in the Zotero APA style.

    If that's not correct, please provide documentation and it's easy to change in the style.
