Single citation used more than once in word is duplicated in Zotero generated bibliography

Hi- I have cited a single reference multiple times in a word document using the Word plug in, and instead of listing all 3 instances as reference 1, it is showing up as references 1, 5, and 6 (though they are all the identical reference). I confirmed this reference is listed only once in my Zotero library. I tried refreshing the citations, closing and restarting word and zotero, unlinking the citations and starting over again, but nothing is working. Zotero is otherwise working fine. I am just driving myself insane trying to fix this problem and am losing too much time on this and need to phone a friend for help.
