Annals of internal medicine help

I am trying to find the reference for submitting to annals of internal medicine. However the annals of internal medicine or any of the NLMs does not fit the reference guidelines indicated on the submission site.

    Yes, sorry for not being descriptive. Annals require when there are more than 3 authors to list them " _____ et al."

    This is an example from one of their recent publications. This is the reference style that they require but I can't seem to find any that fits this one Zotero.
