I generated this by "Add bibliography" in Mac Word. I get the same behavior by selecting them in zotero and using the "Create Bibliography from items" tool. Zotero 4.0.19 and Zotero for Word Mac 3.5.7.
So it appears that the ones listed as "true" are identical. The ones spelled out are slightly different (sometimes just a period or not on the middle initial).
I have edited the style changing:
subsequent-author-substitute="true"
to
subsequent-author-substitute="——"
and it appears to work now. I made the change in github, so it should show up soon.

And are you sure the authors that _don't_ get replaced by "true" are actually entered exactly the same way in Zotero?
