I suppose this might only be an issue when importing existing annotations from PDF files. If you're adding a new note annotation, it can be placed in a white space where it doesn't disturb reading, right?
The annotation-comment icon is just an indicator that there's a comment on the highlight. The note-annotation icon is supposed to be much bigger, because it has to be easily clickable itself. You can argue that it's too big, but it's in line with the size of the note icon in, for example, macOS Preview. (Note also that the icon scales with the page zoom.)

That makes sense to me. However, in iOS (version 1.0.24 Build 222), the note icon appears smaller than the annotation comment icon (that I will cautiously mention again is itself too large)
