Seeking Author (date: page number) format for footnotes/endnotes

I am seeking a format for footnotes/endnotes that includes only:
Author's last name [adding first initial if there is more than one author with the same last name cited in the document] followed by open parenthesis, followed by year of publication [using a letter a, b, c, etc. if the same author published more than one item in the same year] followed by a colon if there is a page number followed by a close parenthesis.
The bibliography should be Chicago Style.

I see that I wasn't clear at the end of my request concerning the page number. Below is what I intended:
Author's last name [adding first initial if there is more than one author with the same last name cited in the document] followed by open parenthesis, followed by year of publication [using a letter a, b, c, etc. if the same author published more than one item in the same year] followed by a colon if there is a page number followed by the page number if there is a page number followed by a close parenthesis.
The bibliography should be Chicago Style, including the a, b, c, etc. next to the years when appropriate.

Generally much easier if you provide specific examples and, ideally, a link to the styleguides.

Thank you so much. I can use World Politics with a slight modification. How do I edit the style so that there is a colon between the year of the publication and the page number rather than a comma?
