Seeking Author (date: page number) format for footnotes/endnotes
I am seeking a format for footnotes/endnotes that includes only:
Author's last name [adding first initial if there is more than one author with the same last name cited in the document] followed by open parenthesis, followed by year of publication [using a letter a, b, c, etc. if the same author published more than one item in the same year] followed by a colon if there is a page number followed by a close parenthesis.
The bibliography should be Chicago Style.

The bibliography should be Chicago Style, including the a, b, c, etc. next to the years when appropriate.
