Zotero doesn't have a watchfolder function and ZotFile doesn't implement it (it used to have a stripped down version of it, but that never worked well and was discontinued in version 5).

I have tried it, and it does not. If I have highlighted text with an associated annotation, Zotfile extracts only the highlighted text, not the annotation.

Also, careful with that -- the Zotero data directory shouldn't sit on OneDrive (if you end up using ZotFile, you can specify a target custom folder on OneDrive, though).

You can use the Zotfile plugin to mimic your Zotero hierarchy, though I personally also recommend just allowing Zotero to Manage your file location and using Zotero to search and navigate.

Zotfile extractions three types of PDF annotations: Highlighted text, Text in comment bubbles, and Underlined text. I don't believe there is a way to easily stop one of those from being extracted.
