Does the style work when you use "Right-Click to Create Bibliography" from within Zotero? (see http://www.zotero.org/support/creating_bibliographies#right-click_to_create_bibliography ) Does the problem also manifest itself in a fresh Word document?

This is a bit technical, but the PubMed XML does contain the journal ISSN, which can be used to match and consolidate title variants of the same journal.

You don't need to export files or notes. Just rename the file extension of the exported file from ".rdf" to ".txt", and it should open in a text editor.

I'm usually the one recommending jEdit. Sublime Text (http://www.sublimetext.com/) is also quite nice (see http://www.marco.org/2012/08/10/next-text-editor for a review). It's a cross-platform, free to evaluate, commercial editor.
