I do not think that Zotero has undo functionality for notes. If you have synced your library online, the old version of the note might be there.

There probably is not. If you have the citation data in Zotero, you can export it in the APA format. If you are more specific about your problem, then it would be possible to give a more specific answer.

What would be nice that if a standalone attachment is selected, Zotero would suggest to create a parent item or to retrieve metadata. The parent item and metadata could even have their own buttons on the attachment details pane.
