No, I can't see that. FWIW, with my book, I did the index myself, with paper and pen. E.g. you get the final proofs, and then you sit down and go through it page by page.

Well, CSL has some magic for dealing with roles, and part of that magic is based on the assumption I mention above. Otherwise, the styles would be a fair bit more complex.
