I’m still not following. What are you using this field for? In general, I would suggest keeping custom information in the Extra field rather than storing it in a field not meant for that purpose.

The APA 7 style correctly formats italics. How exactly are you creating your bibliography? Note that you will generally want to use Zotero’s Word plugin to create automatically updating bibliographies.
