How do you add or edit a citation in Word using Zotero SA?

When I install Zotero SA and open up a new Word document (Word 2008 on MAC) there is no Zotero menu/toolbar to allow me to add a citation - am I missing something? In the preferences/cite pane, the plug-in hyperlink for Word does not work.
