Added new citation style for International Journal of Radiation Oncology Biology Physics

Added new citation style for International Journal of Radiation Oncology Biology modified from the default AMA style.
Please comment as this is my first commit

thanks - looks good.
two things -
first, please add the CC license under which the AMA style is licensed, too (check that style to see how).
second, I just fixed the access macro for the AMA style - you still use the old one - make sure that's correct and else use the one I added in AMA today.

I have deliberately removed the access macro as it is not required by the journal. As far as the CC license is concerned I will add it

great - I had just seen that the macro is still in the style - but since you don't use it - nevermind (you could consider removing it to keep things clean, though.

Made the following changes in the latest commit :
1. Removed Access Macro completely
2. Added rights description as asked.

P.S. When commiting should I commit in the csl or the dependant subfolder.

the csl folder, just like you've done so far.
Dependent styles are styles that are 100% identical to existing styles - if you open one of them, you'll see that they don't actually contain any code - they just have the name of the style and the link to the corresponding independent style.
