Add Note shows a yellow bar and is for adding Zotero notes to your document. If you're trying to add a citation, click Add/Edit Citation.

No, you're misunderstanding what that button does. See the link.
Use Add/Edit Citation for citations and Add/Edit Bibliography for bibliographies.
