Just did the search for you and http://editor.citationstyles.org/styleInfo/?styleId=http://www.zotero.org/styles/apa-old-doi-prefix is an 80% match, so I would use that as a base.

Sorry , I read the instruction but still I need to clarify. Besides the two example , do I need to provide all the reference bibliography of the Mares book chapter and the Campbell & Pedersen journal article? Thanks.

University styles are not a priority though, but I'm happy to push it up on my list for some money.

1. Currently, the references are missing a final/closing period.
2. An extra comma is being inserted between title and edition.

The guidelines you have linked to are for Goldsmith's London (which btw is not accessible) while you posted in a thread for a style for Monash which is a University from Australia.
However, I can see some mistakes with the Monash style and will put in a fix.
