Style Errors: Implementation Science

There seem to be a few errors in the Implementation Science bibliography output:
1) Nothing should be bolded or italicized, regardless of type,
2) A journal article reference should be structured as follows:
Smith JJ. The world of science. Am J Sci. 1999;36(3):234-5.
**Note that no spaces should be added between the year, volume, issue, or page numbers.
3) A book section should include page numbers, the editors, and the full book title as below:
Wyllie AH, Kerr JFR, Currie AR. Cell death: the significance of apoptosis. In: Bourne GH, Danielli JF, Jeon KW, editors. International review of cytology. London: Academic; 1980. p. 251-306.
4) Citations in the reference list should include all named authors, up to the first six before adding 'et al.'.
