Suddenly won't let me manually enter or edit

This program is amazing, but all of a sudden when I try to add a new entry manually, it makes an empty entry but gives me no fields to enter information. I have clicked every button and made sure there are no pop-up blockers on. I am on a Mac (OS X 10.7.5) using Firefox (35.0.1). Can anyone help? THANK YOU!

    Restart Firefox and try again. If that doesn't help, provide a Report ID. (A link to a screenshot might also be helpful, since it's not entirely clear what you mean, but you can start with a Report ID.)
