Style request: [The American Journal of Surgical Pathology]

I think that this style is quite similar to one already created for American Journal of Clinical Pathology, but with some slight differences.

At any rate, here are two example citations:

Campbell JL, Pedersen OK. The varieties of capitalism and hybrid success. Comparative Political Studies. 2007;40:307-332.

Mares I. Firms and the welfare state: When, why, and how does social policy matter to employers? In: Hall PA, Soskice D, eds. Varieties of capitalism. The institutional foundations of comparative advantage. New York, NY: Oxford University Press; 2001:184-213.

Of course for the journal citation, as most of the journals will be indexed in medline, the titles will need to be abbreviated appropriately. Also, I was unable to add in italics in the text here, but the journal and book titles should be italicized.

Citations within the text are numbered, superscripted, and then listed in the bibliography in the order in which they appear.

NLM Title Abbreviation: Am J Surg Pathol
ISSN:0147-5185 (Print) ; 1532-0979 (Electronic) ; 0147-5185 (Linking)

And one articles that is free (at this time):

I understand that these things may take a long time, which is not a problem as I am not in a rush. Thank you in advance for your help!
