When you add a new item, it should show up with the same feed refreshed, yes? I added a new item, synced libraries, and then refreshed that feed. The new item didn't show up. Maybe that is the issue...?

exactly, yes. But might take a little time, I don't know exactly how the API handles that. Also, make sure the sync has gone though and you see that item in your library (and in the collection you've create the feed for if any).
Obviously as long as it doesn't show up in the feed it won't trigger IFTTT
okay, I waited for about fifteen minutes between when I saw the new item in the feed, and waiting for IFTTT to trigger (I read that for most triggers, it's about 15 minutes between each one). No response, so I checked it: and "recipe check fail."

Looked to see this morning whether the recipe had triggered - it didn't, and a forced run failed again.
I did notice, when looking through my Zotero, that an error report had been logged - 31583395. I don't know if this will shed any light on the situation at all, but thought I'd throw it out there.
as long as the item syncs and shows up in the feed, the Zotero error isn't relevant - error reports often pick up unrelated or irrelevant issues. So let's try to set this up for a group I can use, so I can replicate what you're seeing. Could you set up the recipe for https://api.zotero.org/groups/32807/collections/MMNMDX7T/items/top?start=0&limit=25 Let me know when you're done and I'll add an item.
OK, I get the "Recipe Check failed" message now, too, while my old Recipe using the same feed still works. Even if there are no new items, IFTTT should just say "Recipe checked" and not trigger.
I don't think it's on the "then that" side of things - I've stripped this down to just sending an e-mail with the entry title.
At this point this looks like an IFTTT issue, would you mind reporting there? https://ifttt.com/help
