Style Request: British Society for Research into Learning Mathematics

I would be very grateful if you could make a style for our BSRLM Informal Proceedings that I could promote to members. This is also the style needed for the journal Research in Mathematics Education.

The nearest style I have found is The Accounting Review

The differences I have found are:
Citations - none
References -
We never use full first names; whereas The Accounting Review style does for edited books. We use only initials.
For page numbers the style does not repeat leading digits for numbers >20. So for example 225-229 is written 225-9, but 11-14 stays the same.
For web pages we want the date of access too, so after the url there is a space then open parenthesis access-date-with-month-in-words close parenthesis full stop
For a repeated author the 3-em space should not have a full stop after wards.
Thesis: the reference should say "PhD diss.," before the University name
