delimiter before et-al
Journal of Gastroenterology and Hepatology requires the following for author lists:
Nishida T, Kobashi H, Fujioka SI et al.
As far as I can tell, there is no way within CSL 1.0 to achieve this. To quote the specification, 'With a single name (et-al-use-first="1"), the "et-al" term is preceded by a space (e.g. "Doe et al."). With multiple names, the "et-al" term is preceded by the name delimiter (e.g. "Doe, Smith, et al.").'
Would it be possible to have an extra attribute such as 'delimiter-preceeds-et-al'?

This is on track for inclusion in CSL 1.0.1 (it will be "delimiter-precedes-et-al"), whenever that is released.
