Zotero Icon Not Appearing in Word

A student is having difficulty syncing his Zotero and Word documents. The add-on for Word seems was installed, but it does not appear as a plug in in his Word documents.

He is using a Mac (OS X Yosemite version 10.10.4) and Windows for Mac (version 15.13.1).

15.13.1 is very old. He should start by updating to the latest version of Word 2016, currently 16.10 (or possibly 15.37 if this is a volume license).
