Zotero Add-in does not appear in MS Word

For some reason, I can no longer find Zotero in MS Word - it used to be there, but no longer. Zotero in Firefox tells me that the "Microsoft Word add-in is currently installed", but it does not appear in the Word Ad-ins menu.

Zotero 4.0.28.3
Zotero Word for Windows Integration 3.1.20
Windows 7 Professional, SP 1
MS Word 2007

Have you tried re-installing from the Cite tab of the Zotero preferences?

Yes, I have reinstalled ZWforWI - and I've restarted both Firefox and Word.
