Can't add multiple authors

The '+' and '-' icons have disappeared. It only let's me add last/ first name for one author. The only little icon remaining deletes the information for the one author added.
Where did the '+'/ '-' icons go. AND- how do I get them back???

Have you tried hovering the pointer over the last creator?
